Azure LLC was founded in 2003 with one clear mandate: to provide individuals living with intellectual disabilities, autism, traumatic brain injury (TBI), major mental illness, and developmental disabilities with the highest possible quality of housing programs and clinically-based supportive services. Consistent with this focus, we also provide families, caregivers, stakeholders, monitors and referral sources with the information and outcome data from our programs that they need to be assured that individuals are treated with the highest levels of clinical care, compassion and respect. With this charge, we provide these important services based upon our mission, vision and values.
Our Mission
To provide measurably high-quality, individual preference-centered, strengths-based residential and healthcare coordination services in a safe and supportive home-like environment. We not only maintain but also vastly improve the health and well-being of our individuals and support them to become self-sufficient and as independent as possible. In addition, we facilitate and broker expert health assessment services to county, state, regional, national and global entities that require assistance with mental-health disaster planning and assessment services.

We are committed to providing consultation and training services that are specific to the needs of each entity (county or state governments, state human services agencies, international service organizations, non-governmental agencies, local health services planning bodies, and other organizations) in the context of mental health disaster services planning and implementation.

The Azure team realizes that it is a privilege to provide services to our individuals. Each day, we support each person thru individualized programs that meet their needs through an annual planning process that includes all parties who support the individual (e.g. family members, attorneys, guardians, Services Coordinators, Azure’s clinical team, others). Azure implements the plan and ensures that our programs help individuals reach their goals, including monitoring and improving their health status so that they live their lives to the fullest. While these goals do vary based upon the needs of the individual, our desire to provide each person with a supportive, encouraging environment is based upon providing one simple but very important principle — choice. Our focus on choice enables us to provide appropriate goals, programs, services, training, and therapeutic activities based on an understanding of the individual’s needs while maximizing their strengthens and choices.
Affirming the worth of the individual is at the core of all we do at Azure. Assuring the dignity of our individuals and treating our referral partners with respect is the underpinning for both what we do and how we do it. Whether it is including our individuals in the hiring process for new staff or timely follow-up with our referral partners, medical specialists, day programs, and other stakeholders, we clearly understand that all individuals, regardless of their ability, have value and must be treated with the utmost respect.
