District of Columbia Medicaid Waiver Program

For or over 7 years, Azure has proudly served as a certified District of Columbia Home and Community-Based Medicaid Waiver Services provider. These services include (but are not limited to):

 

 

Supported Living

Azure realizes individuals, regardless of disability, desire to have meaningful choice and control of their daily lives. As a provider of Supportive Living services, the Azure team works with individuals to foster optimal independence and community inclusion in an effort to reach their long-term personal goals. Support services may include assistance with activities of daily living, behavioral programs, transportation, integration into community life, and personal financial matters. This level of support is based upon an individualized annual plan and can be increased in times of crisis and decreased as independence grows and milestones are reached.

 

 

Residential Habilitation

Azure is pleased to serve as a residential habilitation service provider. At this level of care, we provide comprehensive services for our individuals including:  housing, behavioral programs, nutritional assessment and individualized menu planning, personal hygiene training, therapeutic activities, transportation and other related services. Our approach is unique because of our commitment to care and excellence demonstrated by our professional direct care staff, a safe and welcoming home environment, and a clinical and administrative management team commitment to excellence. Behavior Support

Azure provides clinical and therapeutic services that foster adaptive behaviors for individuals with intellectualvelopmental disabilities. The clinical focus of our service is the development and monitoring of individualized Behavior Support Plans (BSPs), which provide a framework for individuals’ annual goals. The BSP also allows the clinical staff to track an individual’s progress as well as it provides evidence of individual independence, skill building, and increased inclusion in the community.
 Azure’s clinical team creates, implements, and monitors the plans in order to track individuals’ identified behaviors, behavioral interventions provided by direct care professional staff, and to identify the need for interventions as warranted by the data collected and analyzed. We utilize the services of a Board Certified Licensed Independent Clinical Social Worker, a Registered Nurse, other Masters’ level clinical staffers, and trained direct care professional staff.

 

 

Respite Care

Azure provides short-term respite care for individuals, which incorporates the needs of the individual with clinically appropriate housing services and therapeutic activities during times when a caregiver is unable to provide care. Azure understands the importance of the continuity of care in the lives of individuals, and coordinates respite care services including medication administration, behavior supports, and transportation services seamlessly during the respite period.
